Reflective Patient Safety Organization (RPSO) is a Patient Safety Organization listed under the Patient Safety and Quality Improvement Act of 2005, per the Patient Safety Rule at 42 CFR Part 3. Only AHRQ-listed PSOs can offer the federal confidentiality and privilege protections of the Patient Safety Act. Working with RPSO is voluntary.
Our mission is to improve patient safety and the quality of healthcare delivery through the collection and reflective analysis of de-identified clinical safety reflections, the development of safety recommendations, and direct feedback to healthcare providers.
How safety reflections are submitted
Providers work with RPSO under a written Patient Safety Act agreement. Their clinicians submit safety reflections through an Approved Submission Platform - a secure, RPSO-vetted pathway into RPSO's protected system. RPSO receives reflections only through platforms it has reviewed and approved against its own security and data-handling standards.
Approved platforms function solely as secure conduits - independent of RPSO's analysis, interchangeable, and holding no protected status themselves. RPSO's platform-approval process is open to any platform that meets its criteria. Platforms currently approved include Amplifi and Learner+.

RPSO is listed by AHRQ under the Patient Safety Rule (42 CFR Part 3). Only listed PSOs can confer its federal privilege and confidentiality protections. Verify our listing - PSO No. P0293 - in the AHRQ directory.

Clinicians submit reflections through an RPSO-approved submission platform. The platform is a secure conduit only; privilege and confidentiality attach when a reflection is reported into RPSO's protected system - never to the platform itself.

Clinicians reflect on a safety event in their own words and choose to submit it as a patient safety report. Clinical and Practice Reflections - not incident forms - are how safety insights reach RPSO.
